Job Title: Family Support Case Manager
Weekly Hours: Sunday through Thursday, 8 hours per day, time varies based on needs of caseload.

Job Summary:
The Family Support Case Manager is a liaison between the Department of Developmental Services and the families we serve. The Family Support Case Manager will help develop a family support plan and complete assessments to determine the needs of a family, both in the home and the community. The Family Support Case Manager will lead the implementation of the family support plan as well as assist the family in applying for MassHealth, Social Security, food stamps and other public benefits. The Family Support Case Manager will also attend meetings and advocate on behalf of the family.
Essential Job Functions:
- Provides case management to families through the Family Support Center including assessments, short term and extended navigation services, financial allocation support and resource assistance
- Assists families in the development and implementation of Family Support Plans including; assessments, goals and objectives.
- Provides training to family members utilizing intervention strategies in home/community
- Serves as liaison between family and the Department of Developmental Services as needed
- Provides assistance in locating community resources and referrals as appropriate
- Act as liaison between family and other service providers such as special education programs, health care providers and other community resources
- Maintains timely and accurate documentation according to program and funding requirements
- Provide a range of services, including information, referral, activities, and trainings, along with providing support to families to enable individuals with disabilities to remain in their own homes
- Facilitates various social and recreational groups for families through the Family Support Center or via Zoom as needed
- Conduct outreach on a regular basis to develop community partnerships
Qualifications: Education & Experience Required:
- High School Diploma or equivalent
- Understanding of supports and services needed by families in caring for disabled members.
- Two years’ experience working with individuals with developmental disabilities.
- Experience implementing behavior plans.
- Maintain CPR/First Aid certifications, Safety-Care training, agency-required trainings including online RELIAS trainings and those requested by the Director of Family Support/SSSS.
- Excellent listening and communication skills
- Proficient using Microsoft Office and able to learn other office computer software.
- Must have a current, valid state issued driver’s license.
- Employees are subject to driver’s license record checks and must maintain an acceptable driving record, valid motor vehicle insurance and registration.
- Pass background and reference checks.
- Fingerprinting required
Physical Demands/Work Environment:
- Must be able to lift up to 40 pounds
- Must be able to assume and maintain a variety of postures (kneeling, squatting crawling, sitting, and standing) for extended periods of time.
- Maintain all files in a confidential manner
Reports Directly To: Director of Family Support
Professional Development/Training Offered:
- Hold and maintain certifications in CPR/First Aid & Safety-Care
- 20 hours of training (both live and online) annually as assigned by supervisor.
Pay: $23.00 - $25.00 per hour

Experience:
- Applied behavior analysis: 1 year (Preferred)
License/Certification:
