Field-Based Certified Home Health Aide

Job Summary
We are seeking a dedicated and compassionate Field-Based Certified Home Health Aide to provide exceptional in-home care to clients across diverse settings. As a vital member of our healthcare team, you will deliver personalized assistance that promotes independence, safety, and well-being. This role offers an exciting opportunity to work directly with individuals requiring support due to age, disability, or illness, ensuring they receive the highest quality of care in their own homes and communities. Your expertise will help improve lives through compassionate caregiving, meticulous attention to medical and personal needs, and fostering a safe environment for all clients.

Duties

  • Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using tools such as Hoyer lifts and other assistive devices
  • Support clients with specialized needs such as Alzheimer's care, dementia care, memory care, hospice support, or working with individuals with developmental disabilities or disabilities
  • Maintain a clean and safe environment by assisting with light housekeeping tasks such as laundry, cleaning, meal preparation, and ensuring infection control standards are met
  • Document all patient interactions thoroughly in electronic medical records (EMR systems), ensuring compliance with healthcare regulations and standards
  • Communicate effectively with healthcare professionals, family members, and caregivers to coordinate care plans and report any changes in condition or concerns

Experience

  • Valid Certified Home Health Aide (CHHA) certification or equivalent from an accredited program
  • Proven experience providing home health or long-term care services in a home or community setting
  • Prior nursing home experience or assisted living background highly preferred
  • Familiarity with HospiceMD for accurate documentation of patient records and medical history
  • Experience working with individuals with dementia, Alzheimer's disease, developmental disabilities or in hospice environments is advantageous
  • Knowledge of medical terminology relevant to home health caregiving practices
  • CPR certification required
  • Demonstrated ability to perform patient observation accurately and respond swiftly to changing conditions
