Flexible Physician Opportunity (Full Time & Part Time Availability)

At Pembroke Medical Associates, we are committed to providing exceptional post-acute care services to help patients recover and transition from a hospital setting to a post-acute care setting. Our team of specialized medical professionals have extensive experience in managing complex medical conditions and helping patients achieve their highest level of function and independence.

Some of our services include short stay patient care management, long term care, SNF care, wound care, and complex care management. We work collaboratively with acute care hospitals, post-acute care facilities, and with primary care providers to ensure a seamless transition of care. Through our local connections, we make sure to utilize all available resources and knowledge to best care for our patients.

Pembroke Medical Associates is looking to hire 2 Physicians to join our team and serve the Quincy/Weymouth Area.

A skilled facility/long term care physician provides comprehensive medical care to individuals residing in skilled nursing facilities and long-term care facilities. They are responsible for diagnosing and treating acute and chronic medical conditions, managing medications, coordinating care with other healthcare professionals, and promoting overall health and well-being of the patients.

Key Responsibilities:
1. Conducting initial assessments including history & physical evaluations and periodic evaluations of patients' medical conditions.
2. Developing personalized care plans based on patients' unique needs and medical history.
3. Diagnosing and treating acute illnesses, infections, and injuries.
4. Managing chronic conditions such as diabetes, hypertension, and heart disease.
5. Prescribing and adjusting medications as necessary.
6. Ordering and interpreting diagnostic tests, such as laboratory work and imaging studies.
7. Collaborating with interdisciplinary teams, including nurses, therapists, and social workers, to ensure coordinated care.
8. Providing education and counseling to patients and their families regarding their medical conditions and treatment options.
9. Monitoring and addressing patients' physical and mental health needs, including pain management.
10. Conducting regular visits to assess patients' progress and address any changes in their condition.
11. Adhering to all regulatory and compliance standards related to long-term care facilities.
12. Participating in quality improvement initiatives and ensuring the highest standard of care is provided.

Requirements:
1. Medical degree (MD or DO) from an accredited medical school.
2. Board certification in internal medicine, family medicine, or geriatric medicine.
3. Valid state medical license and DEA registration.
4. Experience and knowledge of skilled nursing facility or long-term care settings perferred.
5. Strong clinical skills, including the ability to diagnose and treat a wide range of medical conditions.
6. Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare professionals.
7. Ability to work independently and make informed medical decisions.
8. Proficiency in electronic medical records and other healthcare software.
9. Commitment to lifelong learning and staying updated with the latest medical advancements.
10. Strong ethical and professional conduct, maintaining patient confidentiality and privacy.
