Foster Care Case Manager

We are looking for a Case Manager that will change the lives of children placed in our licensed foster homes. You will be responsible for protecting the health, safety, and well-being of children and provide clinical and technical support to foster parents through case management, home visits, weekly contact, monitoring of service compliance, documentation, ongoing training and advocacy. The main goal for this position is to effectively work towards achieving permanency for children while actively assessing the needs of the child and the foster parent and coordinating interventions when necessary.

The ideal candidate has a Bachelor's Degree in a human services or related field and a minimum of 2 years related experience. Excellent interpersonal and communication skills, strong time management and multi-tasking skills, and good computer skills are required. Position requires some on-call rotation and the ability to maintain a high level of confidentiality.

The Case Manager will have the following general Responsibilities:

• Meet the physical, emotional, and social needs of foster children placed in Open Hearts CFS licensed foster homes.
• Participate in regular supervision and support with the client and foster parents during frequent home visits, office visits, and telephone contact.
• Complete accurate and thorough documentation for case according to Open Hearts CFS guidelines.
• Provide individual, family, and group counseling, and crisis intervention.
• Develop, monitor, and review with clients their Plan of Service designed to increase strengths, address needs, and reduce conflict in the home.
• Assist in the screening and licensing of potential foster parents.
• Complete home studies for potential foster parents.
• Facilitate training for foster parents to provide them with continuing education.

Job Type: Full-time
