Foster Care Case Manager - Nashville

Job Overview
As a Case Manager, you will play a crucial role in supporting individuals in need by providing guidance, resources, and advocacy. This position requires a compassionate and dedicated individual who is committed to making a positive impact on the lives of others.

Duties
- Assess clients' needs and develop individualized care plans
- Coordinate and monitor services to ensure clients' well-being and progress
- Advocate for clients' rights and access to necessary services
- Collaborate with healthcare providers, social workers, and other professionals to support clients
- Maintain accurate and confidential client records
- Provide crisis intervention and support as needed

Experience
- Experience in childcare, child welfare, or working with people with developmental disabilities is preferred
- Knowledge of child protective services and social work practices is beneficial
- Ability to work effectively with children and families
- Strong communication and interpersonal skills
- Willingness to accept feedback and continuously learn and grow

Job Type: Full-time

Pay: $48,000.00 - $50,000.00 per year
