Summary

We are seeking a Hospice Nurse Practitioner to join our compassionate team in providing exceptional end-of-life care. Located in Greenfield Wisconsin, this role is essential in ensuring that our patients receive holistic support during their most critical times, aligning with our mission of comprehensive care.

Responsibilities

• Conduct thorough patient assessments to develop individualized care plans.
• Administer medications and treatments as prescribed.
• Collaborate with interdisciplinary teams to ensure optimal patient outcomes.
• Provide education and support to patients and families regarding hospice services.
• Monitor patient progress and adjust care plans as necessary.
• Maintain accurate medical records and documentation.
• Participate in case management and discharge planning.
• Advocate for patients' needs within the healthcare system.

Qualifications

• Master's degree in Nursing with a focus on hospice or palliative care.

Requirements

• Current Nurse Practitioner license in the state of Wisconsin.
• Experience in hospice & palliative medicine is highly preferred.
• Strong assessment and clinical skills are essential.
• Must be a licensed driver who can travel to all business locations.
• Excellent communication and interpersonal skills.
• Ability to work collaboratively within a healthcare team.
