We're seeking a skilled and empathetic Full Time Hospice Case Manager to join our dynamic team in the Flint and surronding areas. Key Responsibilities:

• Conduct initial and ongoing assessments of patients' needs to develop comprehensive care plans
• Provide emotional support and education to patients and families about end-of-life care and self-care techniques
• Collaborate with the interdisciplinary team to ensure high-quality care and regular updates on patient progress
• Supervise and mentor nursing staff and paraprofessionals in delivering exceptional care
• Maintain accurate clinical documentation and participate in performance improvement initiatives
  
  

Requirements

• Current Michigan Registered Nurse (RN) License in good standing
• Minimum of 1 year experience in hospice or palliative care
• Strong assessment and critical thinking skills
• Excellent communication and interpersonal skills
• Ability to work collaboratively in a team environment
• Empathetic and compassionate approach with patients and families
• Willingness to participate in on-call duties as needed
