Position: Full-time RN Health Coordinator
Shift/schedule: Tuesday-Saturday / 7:00a-3:30p
Pay Range: $34.00/hr to $42.00/hr

Position Summary: Under the guidance of the Medical Director, responsible for the provision of nursing services to the residents.
Education and Training: NYS Registered Nurse License. Minimum of two (2) years of relevant experience, including knowledge of population. CPR/First Aid Instructor.

Essential Duties:
- Responsible for provision of nursing services to residents.
- Conducts admission process for prospective residents.
- Conducts and interprets comprehensive nursing assessments and mental health exam.
- Develops measurable treatment plan objectives based on the Strengths, Needs, Abilities, Preferences (SNAP) Model, as well as initial and ongoing medical assessment.
- Initiates weekly medical progress notes and treatment plan review note and clearly documents all medical treatment received.
- Contributes to aftercare plan development relative to ongoing medical needs.
- Conducts didactic lectures on topics related to health and addiction.
- Ensures the provision of education, risk education, counseling and referral services to all patients regarding HIV and AIDS, tuberculosis, hepatitis, sexually transmitted diseases and other communicable diseases.
- Assists with the education of residents and staff regarding psychotropic medications, potential side effects, and related medical concerns.
- Delivers CPR training to SJATRC employees.
- Adheres to Pharmacy Services Policy in its entirety, inclusive of controlled substance policy, procedures, and monitors.
- Actively participates in meetings, committees, and quality assurance activities.
- Responsible for additional duties as may be assigned by the Program Director.
- Has knowledge of medical and psychiatric issues specific to population.
- Assess and monitor residents experience signs and symptoms of withdrawal using appropriate withdrawal scales.
