Reports To: Senior Director of Programs and Services

Job Status: Monday-Friday 8:00am-4:00pm

Location: Westbury, NY (LIAD Center)

LIAD Center History: Founded in Port Washington in 1988, LIAD is now located in Westbury, NY and provides resources, programs, and support from pre-diagnosis through end-of-life for individuals with Alzheimer’s disease and their families, throughout Long Island.

Responsibilities and Duties

A) Caregiver Support Groups and Education

a. Facilitate caregiver support groups (virtual/In-person) for spouses, adult children, and other family caregivers of those with Alzheimer’s and related dementias. Virtual and in-person groups at the LIAD Center in Westbury. Currently, there are five weekly groups and one monthly, one annual 8-week bereavement group: subject to change.

b. Manage, schedule and facilitate monthly educational seminars and webinars; plan educational seminars with community partners and guest speakers; facilitate educational programming in the community such as libraries and senior centers.

c. Schedule and facilitate social programming for family caregivers twice a month, and quarterly for former caregivers.

B) Caregiver Case Management

a. Manage a caseload of 15-20 caregivers through Nassau County’s National Family Caregiver Support Program (annual assessments, quarterly check-ins, referrals and reporting). Caseload subject to change.

b. Attend required meetings and training with Nassau County Office for the Aging, and New York State’s Office of the Aging (virtual).

c. Conduct and manage initial assessments for Parker Jewish’s Willing Hearts, Helpful Hands Caregiver Support Program (grant funded)

C) Manage Caregiver Call Log

a. Responsible for taking caregiver phone calls and providing resources, referrals, and program information.

b. Delegate referral calls to the other social workers and interns, as needed for assistance.

c. Delegate to other social workers as needed.

D) General Social Work Responsibilities

a. As needed, assist with intake assessments, walk-ins and tours, in conjunction with Director of Client Coordination and Director of Day Program Services.

b. As needed, assist with in-home intake assessments for In-Home Respite Program.

c. As needed, assist Director of Day Program Services with annual care plans for participants and quarterly notes.

d. If the Day Program Director is out or unavailable, this position may be asked to cover the day program Monday-Saturday if needed.

e. Assist in other tasks and projects, as requested by Senior Director of Programs and Services.

f. As needed, provide tasks to social work interns. Supervise a social work intern, if SIFI certified.

g. Attend staff meetings, outreach activities and special events after hours or weekends as needed.

h. Assisted at 4 mandatory fundraisers events each year, and others, as needed.

Qualifications

Required: LMSW and 3 years of related work experience, or LCSW. Excellent clinical, organizational, oral, and written communication skills. Ability to conduct in-home assessments, and experience in facilitating support groups. Ability to do public speaking and presentations virtually and in the community. Experience with dementia, or the elderly population, is strongly preferred. Attending evening fundraisers and community events several times a year is required.

Salary Range: $64,000-$68,000, or based on experience (more or less)
