We are seeking a highly organized and compassionate Functional Medicine Program Coordinator to support patient care and program operations. This role is responsible for guiding patients through their wellness journey, ensuring smooth onboarding, consistent follow-ups, and strong engagement throughout the program.

You will act as the key connection between patients and the clinical team—helping patients stay on track, informed, and supported throughout their personalized functional medicine plan.

Key Responsibilities

• Manage patient onboarding into functional medicine programs
• Handle patient communication via phone, email, and messaging
• Schedule appointments and coordinate follow-ups
• Track patient progress, labs, and program milestones
• Support patient adherence to wellness plans (non-clinical guidance)
• Maintain accurate records in EMR/CRM systems
• Coordinate with providers, health coaches, and clinical staff
• Ensure high patient engagement, satisfaction, and retention

Qualifications

• Experience as Medical Assistant (MA), Care Coordinator, or Health Coach preferred
• Strong communication and interpersonal skills
• Highly organized with attention to detail
• Comfortable speaking with patients regularly
• Experience with EMR, scheduling systems, or CRM tools
• Interest in functional medicine, integrative health, or wellness care

Preferred Experience

• Functional or integrative medicine clinic experience
• Concierge medicine or wellness program experience
• Patient enrollment or patient success role experience
• Background in chronic disease, autoimmune, or gut health support
