-Base Guarantee: $1,500/day (8-hour day)structured as a draw vs collections

-35% of adjusted production

-Health Insurance: 50% of premium paid by Company

-401K + Company contribution: Eligibility after 1 year of employment

-Continuing Education: $3,000 annual stipend (includes ADA dues, DEA license)

-Malpractice Insurance: Premiums 100% covered by employer --

-Relocation Assistance: $10,000 (reimbursed with receipts, payable within 60 days)

-Sign-On Bonus: $10,000 ($5,000 on start, $5,000 after 90 days; prorated repayment if departure <1 year
