APRN – Geriatric Primary Care
Location: Fairfield County, CT
Schedule: Full‑time
Setting: Home‑based primary care / ALF / community geriatric practice
Position Summary
The APRN provides high‑quality primary care to an assigned panel of geriatric patients, focusing on chronic disease management, preventive care, and continuity. This role is ideal for a clinician who values autonomy, relationship‑based care, and a collaborative team environment.
Key Responsibilities
- Deliver comprehensive primary care to older adults in home‑based or assisted‑living settings
- Perform routine visits, acute visits, and follow‑up care
- Manage chronic conditions (CHF, COPD, diabetes, dementia, hypertension, etc.)
- Conduct annual wellness visits, medication reviews, and preventive screenings
- Coordinate care with physicians, specialists, home health, and family caregivers
- Document all encounters in the EMR with accuracy and timeliness
- Participate in interdisciplinary case reviews and quality‑improvement initiatives
- Provide patient and family education to support long‑term health outcomes
- Maintain compliance with state and federal regulations
Qualifications
- Active Connecticut APRN license
- Board certification in Adult-Gerontology Primary Care or Family Practice
- DEA registration (or eligibility)
- Experience in geriatrics, primary care, home‑based care, or internal medicine preferred
- Strong clinical judgment and ability to work independently
- Excellent communication and documentation skills
