Job Summary
As Clinical Director, you will manage and lead our Care Management practice and provide clinical support across our business teams. Your focus is on the safety and well-being of our clients as well as meeting the day-to-day needs of a growing team. A successful candidate will use a collaborative approach, understand the big picture of our business and be able to pivot and prioritize, as needed.

Essential Functions & Responsibilities
1. Manage and lead our Care Management team of 6 to ensure they provide maximum value and support to clients
- Train new hires, shadow team members, and provide coverage for team member vacations and other time away from office
- Collaborate closely with CEO to provide guidance to our team members
2. Provide clinical support to all Care Management, Life Enhancement and Business team members, as needed
3. Review Care Plans, quarterly summaries, and court related documents, for clinical accuracy and client probate matters for clinical and legal accuracy
4. Up to 25% of time dedicated to providing Care Management service to clients.
-* Essential Functions & Responsibilities*
- Clinical Advocacy & Care Coordination
- Comprehensive Assessment: Conduct detailed assessments and summaries of findings to identify client needs and ensure high-quality care delivery.
- Care Planning: Develop, implement, and update goal-oriented, written care plans.
- Family Liaison: Communicate regularly with family members, navigating complex family dynamics while advocating for the client's best interests.
- Medical Oversight: Coordinate medical appointments, ensure medication compliance, and oversee care provided in both home and facility settings.
- Crisis Intervention: Identify potential risks and implement proactive solutions to avoid crises; provide immediate intervention when emergencies arise.
- Placement & Transition Management
- Facility Coordination: Assist families with placement and transitions to/from assisted living or nursing home environments.
- Monitoring: Conduct routine visits to clients in private homes or facilities to ensure they are safe, nourished, and receiving proper care.
- Specialized Support: Manage communication and care plans throughout the conservatorship process and provide long-range planning for those not yet in need of active care.
-* Resource & Expert Referral*
- Community Expertise: Maintain extensive knowledge of local eldercare resources, social services, and mental health programs.
- Expert Network: Review financial, legal, or medical issues and refer clients to approved experts when necessary.
- Caregiver Support: Screen, arrange, and monitor in-home help while following all licensing and labor law guidelines.
- Administrative & Operational Excellence
- Documentation: Enter all client interactions and daily billable time into the client management database immediately.
- Reporting: Forecast monthly billable hours and provide regular updates to the Office Manager.
- Compliance: Follow all About Senior Solutions processes, forms, and systems to maintain operational consistency.
- Brand Ambassadorship: Market services and create goodwill for About Senior Solutions within the professional community.

6. Periodic travel to all locations
Qualifications & Requirements
- A graduate degree in one of the following programs is highly desirable: MSW, RN, MFT, MA Gerontology, MA Psychology or MA Occupational Therapy. Fiduciary license or law degree a plus
- Background in malpractice and/or fiduciary work
- Experience in care or case management of older adults
- Experienced in dementia care
- Experience working with mental health issues and coordinating services for clients with mental illness
- Experience working with elder abuse matters and reporting
- Knowledge of local eldercare resources a plus
- Experience working with disabled adults and coordinating services for clients under 65 with disabilities a plus
- Excellent written and oral communication skills
- Strong organization and time management skills
- Proficiency in Google Workspace and Microsoft Suite of products
- Available Mon-Fri during business hours and on-call outside of business hours, as needed
- Valid California driver’s license, insurance, personal transportation
