Geriatric Long-Term Care Licensed Clinical Social Worker

We are seeking talented and experienced full-time Social Workers who spend 100% of their time providing resident care in long-term care health facilities. The ideal candidate for this position will have previous experience working with the geriatric population. Our goal is to reinvent the long-term care experience through innovative training programs for our Social Workers that encourage a dynamic and effective interpersonal relationship between residents, families, and service providers.

In this role, you will provide meaningful care to residents through providing cognitive behavioral therapy. Additionally, you will aid residents and families in the Advance Care Planning process and serve as a palliative care resource.

Primary Responsibilities:

• Conduct resident assessments of psychosocial and living needs
• Identify individual social and emotional needs of residents
• Develop plan for client services oversight and coordination
• Participate in Advance Care Planning meetings with residents, family, and staff; Integrate goals discussed into clinical approach with resident
• Provide emotional support and counselling to palliative care residents and their families
• Participate in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident
• Collaborate with nursing staff and facility leadership

Required Qualifications:

• Licensed Clinical Social Worker (Required)
• A clinical background in geriatric, adult, or family medicine specialty
• Past experience working in a nursing home or with seniors in other clinical settings
