Senior Moments Care is a concierge-style geriatric house call and primary care practice dedicated to enhancing the quality of life for older adults. Guided by the Geriatric 5Ms (Mind, Mobility, Medications, Multicomplexity, and What Matters Most), we provide compassionate, patient-centered care in the home and in our newly developed Geriatric Center of Excellence in Somers, NY.

Our model blends medical care, cognitive health programming, and caregiver support—creating a truly innovative and holistic approach to aging.

Position Overview

We are seeking a compassionate and motivated Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our growing team. This role offers a unique hybrid opportunity to provide care in both:

• Patient homes throughout Westchester County
• Our Somers-based Geriatric Center of Excellence

You will work closely with a collaborative, mission-driven team focused on improving the lives of older adults—especially those who are frail, homebound, or living with cognitive decline.

Key Responsibilities

• Provide comprehensive primary care to geriatric patients in home and office settings
• Conduct assessments, diagnoses, and develop individualized care plans
• Manage chronic and acute conditions with a focus on quality of life
• Perform medication reconciliation and deprescribing when appropriate
• Collaborate with caregivers and families on care goals and planning
• Participate in advanced care planning and goals-of-care discussions
• Document visits in EMR in a timely and accurate manner
• Support and participate in innovative programs (e.g., cognitive stimulation therapy, caregiver support initiatives)

Qualifications

• Licensed Nurse Practitioner (NP) or Physician Assistant (PA) in New York
• Board certification required
• Strong interest in geriatrics and community-based care
• New graduates are welcome and encouraged to apply (training provided)

Preferred (not required):

• Experience in geriatrics, dementia care, or palliative care
• Comfort working independently in home settings
• Bilingual (especially Spanish) a plus
