Geriatric Nurse Practitioner

Join our dynamic healthcare team as a Geriatric Nurse Practitioner, where your expertise will directly impact the quality of life for older adults. In this vital role, you will provide comprehensive primary and specialized care tailored to the unique needs of seniors, promoting independence, managing chronic conditions, and enhancing overall well-being. Reimbursement is based on billed CPT codes

Duties

  • Conduct thorough patient assessments, including vital signs, physical examinations, and detailed medical histories focused on geriatric populations.
  • Develop and implement individualized care plans that incorporate medication management, health coaching, and preventive strategies aligned with best practices in geriatrics.
  • Manage acute and chronic conditions such as dementia, Alzheimer's disease, osteoporosis, and cardiovascular issues through evidence-based interventions.
  • Perform procedures including suturing, IV infusion, catheterization, spinal taps, and dermal filler administration with precision and sterile technique.
  • Utilize advanced electronic health record (EHR) systems like Epic or eClinicalWorks to document patient encounters accurately and efficiently.
  • Coordinate with multidisciplinary teams on discharge planning, post-acute care, hospice & palliative medicine, and assisted living facilities to ensure seamless patient transitions.
  • Engage in telehealth consultations to extend geriatric care beyond traditional settings while maintaining compliance with HIPAA regulations.
  • Participate in clinical research initiatives related to aging health issues and contribute to quality improvement efforts following NCQA standards.

Qualifications

  • Valid Nurse Practitioner license with certification in geriatrics or family practice; additional certifications in hospice & palliative medicine or behavioral health are a plus.
  • Demonstrated experience working with seniors in settings such as nursing homes, assisted living facilities, or outpatient clinics.
  • Proficiency with EMR/EHR systems including Epic, Cerner, or Athenahealth; experience with telehealth platforms is preferred.
  • Strong knowledge of physiology, anatomy, pharmacology (including CPT coding), and diagnostic evaluation specific to older adults.
  • Skilled in performing complex procedures such as dermal fillers, spinal taps, venipuncture, sterile processing techniques, and airway management.
  • Critical care experience from ICU or Level I/II trauma center environments enhances your ability to handle urgent geriatric cases effectively.
  • Excellent patient assessment skills combined with compassionate communication tailored for diverse populations including children and individuals with disabilities.
  • Knowledge of Workers' Compensation law and infection control protocols ensures comprehensive patient care delivery.
