Duties
- Conduct thorough physical examinations and comprehensive patient assessments focused on geriatric health concerns.
- Manage chronic conditions common among older adults such as hypertension, diabetes, dementia, and arthritis through evidence-based interventions.
- Educate patients and families on health coaching topics like nutrition, fall prevention, medication adherence, and discharge planning.
- Ensure compliance with HIPAA regulations while maintaining meticulous medical records and documentation standards.
Experience
- Valid Nurse Practitioner license with certification in geriatrics or family practice preferred.
- Proven clinical experience working with seniors in settings such as nursing homes, assisted living facilities, or home care environments.
- Demonstrated expertise in acute care scenarios including ICU or Level I trauma center experience; familiarity with emergency medicine protocols highly valued.
- Hands-on experience with medical procedures such as phlebotomy, venipuncture, sterile processing techniques, spinal taps, and airway management.
- Strong background in managing complex cases involving chronic disease management, acute pain management, dialysis procedures, and occupational health considerations.
- Proficiency in EMR/EHR systems like Cerner or Athenahealth; experience with CPT coding for billing purposes is advantageous.
- Knowledge of infectious disease protocols including infection control practices; experience with hospital medicine or surgical units is a plus.
- Skills in utilizing telehealth platforms to deliver remote patient assessments and follow-up care effectively.
