Geriatric Nurse Practitioner - Eau Claire WI

Sequoia Integrative Medical Services LLC is seeking a compassionate and motivated Family Nurse Practitioner to join our growing healthcare team in the Eau Claire area. This position focuses on providing high-quality primary care services within geriatric and long-term care settings, with an emphasis on patient-centered care, prevention, and improving quality of life.

What You’ll Do

• Conduct patient assessments, physical exams, and routine follow-up visits
• Diagnose and manage acute and chronic health conditions, including diabetes, hypertension, and asthma
• Complete admission history and physicals
• Perform consult and discharge medication reconciliation
• Provide preventive care and patient education focused on wellness and disease prevention
• Collaborate with nursing staff and interdisciplinary care teams
• Support patients with behavioral health needs through compassionate, whole-person care

Qualifications

• Master’s or Doctorate degree in Nursing
• Current Family Nurse Practitioner certification
• Active Wisconsin Nurse Practitioner license
• Strong communication and clinical decision-making skills
• Experience in primary care, geriatrics, or long-term care preferred, but motivated new providers are encouraged to apply
