About the Role

As a key member of our innovative care team, you’ll deliver comprehensive primary care services to geriatric patients in their homes and community facilities. This role is designed for maximum flexibility—no on-call duties, no hospital rounds, and full control over your schedule. You can set your own pace and conduct visits at the times that work best for you.

Our care model reduces administrative tasks so you can focus on what truly matters: building meaningful relationships and improving patient outcomes. You’ll act as a clinical leader, collaborating with families and multidisciplinary healthcare professionals to ensure proactive, coordinated care.

Key Responsibilities

  • Provide routine, preventive, and urgent medical care to elderly patients
  • Manage chronic illnesses and evaluate home and living environments
  • Educate patients and caregivers about health conditions, medications, and care plans
  • Create and update individualized care plans tailored to patient needs
  • Work collaboratively with nurses, social workers, and therapists to reduce hospitalizations and enhance quality of life
  • Complete timely and accurate documentation using our streamlined EHR system

Qualifications

  • M.D. or D.O. from an accredited medical school
  • Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Emergency Medicine (preferred)
  • Active California medical license
  • Valid DEA registration
  • Excellent communication skills and a compassionate, patient-first approach

Job Type: Part-time

Pay: $250,000.00 - $300,000.00 per year
