About the Role

As an integral member of our innovative care team, you’ll provide comprehensive primary care services to geriatric patients in home-based and community facilities.

This position is designed for maximum flexibility — no on-call duties, no hospital rounds, and complete control over your schedule. You’ll have the freedom to set your own pace and conduct visits when it best fits your lifestyle, while focusing on building meaningful, long-term relationships with patients and families.

Our streamlined systems minimize administrative tasks so you can dedicate your time to delivering exceptional care and improving patient outcomes.

Key Responsibilities

  • Provide routine, preventive, and urgent care to elderly patients in home and community settings
  • Manage chronic conditions and assess home environments for safety and support needs
  • Educate patients and caregivers on conditions, medications, and treatment plans
  • Develop and update individualized, proactive care plans
  • Collaborate with nurses, social workers, and therapists to improve care coordination and reduce hospitalizations
  • Document visits accurately and efficiently using our user-friendly EHR system

Qualifications

  • M.D. or D.O. from an accredited medical school
  • Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Emergency Medicine (preferred)
  • Active California Medical License
  • Current DEA registration
  • Strong communication skills and a compassionate, patient-focused approach

Job Type: Part-time

Pay: $250,000.00 - $300,000.00 per year
