Position Overview
We are seeking a full-time Nurse Practitioner or Physician Assistant to join our growing team. In this role, you will manage a panel of approximately 100 patients and provide in-person primary care services directly at senior living communities. This is an ideal position for a provider who values meaningful patient relationships, autonomy, and impactful work in geriatrics.

Schedule

• Monday to Friday (Flexible hours)
• Optional nights, weekends, or on-call shifts for additional pay

Key Responsibilities

• Provide comprehensive care to a panel of approximately 100 senior patients
• Conduct physical exams and manage both chronic and acute conditions
• Order and interpret labs, imaging, and other diagnostic tests
• Develop and manage personalized treatment plans
• Educate patients and families on care and wellness strategies
• Collaborate with interdisciplinary teams and facility staff
• Participate in community outreach and education
• Accurately document visits using ICD-10 and HCC coding in the EHR
• Engage in end-of-life and palliative care discussions as needed

Qualifications

• Active California license as an NP, PA, MD, or DO
• Minimum of 1 year experience in geriatrics, internal medicine, or primary care (preferred)
• Strong skills in clinical judgment, patient communication, and care coordination
• Experience with palliative care and telehealth platforms is a plus
• Valid CA driver’s license and access to reliable transportation

• Base Salary: $132,000 – $170,000/year
• Total Compensation: $170,000+/year with bonus potential
