Dynamic and growing company is seeking a compassionate, reliable, motivated and experienced Guardian to join our team. As a Guardian you will provide personal and healthcare advocacy for individuals with developmental disabilities, physical disabilities, mental illness, and the elderly. This role is essential in ensuring the safety, dignity, and well-being of vulnerable adults. Guardians serve a caseload of approximately 30 clients, most of which are in the 7 county metro area.
Responsibilities Include:
- Acting in the best interest of assigned individuals, making informed decisions regarding healthcare, living arrangements, and daily needs;
- Coordinating medical care, appointments, and services;
- Working with individuals and families to support the needs of each individual;
- Working closely with the courts and completing all required court paperwork;
- Advocating for individuals in the area of personal care, health care, and other eligible services;
- Working with case managers and health care professionals to ensure appropriate living arrangements and medical services;
- Seek out and apply for eligible benefits and services for each individual;
- Providing authorization for medical, housing, vocational, and other support services;
- Assist with the oversight of financial assets;
- Conduct regular in-person visits and welfare checks
Qualifications:
- Bachelor's degree in Social Work, Human Services, Psychology, or related field (preferred)
- Previous experience in caregiving, case management, or guardianship strongly preferred;
- Experience with similar client populations;
- Must possess strong problem solving and decision making skills;
- Strong organizational and time management skills;
- Excellent communication and interpersonal skills;
- Must be dependable, accurate, and be able to meet deadlines;
- Ability to handle sensitive information with discretion
- High level of professionalism and reliability.
- Must pass DHS and BCA background study and have a valid driver's license; a Motor Vehicle Check is required.
