Guardianship Case Manager

HOURLY RATE: up to $23.18 per hour


SUMMARY

Come join us as we take on the challenges and opportunities of longer life by safeguarding vulnerable older adults from abuse. Our Lifespan Guardianship program provides a mechanism for personal and financial decision making when older adults no longer have capacity and when the adult lacks a trusted ally to make decisions on their behalf. Under general direction, this position is responsible for providing case management and guardianship services to vulnerable adults age 60 and older and when Lifespan has either been appointed by the court as the legal guardian or Monroe County Adult Protective Services has been appointed and delegates Guardianship case management to Lifespan. This active position is responsible for making personal and financial decisions on behalf of assigned clients. the case manager typically carries a caseload of between 20-25 clients. Responsibilities of this position include a minimum of four client visits during a calendar year and monthly visits when APS is the appointed guardian. Service meetings occur within the client’s home, during medical practice office visits, at hospitals, skilled nursing facilities, and rehabilitation centers or other community establishments. The case manager conducts virtual and telephonic contact with clients, family members and third parties. The case manager may be required to appear in court, provide crisis intervention as needed and is responsible for emergency after- hours on-call rotation. The case manager meets with the Lifespan Guardianship team on a regular basis to review the status of program clients and to engage in joint decision making and problem solving in individual cases.

DUTIES AND RESPONSIBILITIES

1.0 Provide Guardianship case management to ensure clients are served properly.

  • Provide Guardianship care management services in accordance with National Guardianship Association standards, court appointment orders, Lifespan policy and procedures and ethical decision-making standards.
  • Prepare and compile comprehensive personal court reports, activity reports, and monthly summary reports. collect data and statistics as required. Coordinate and act on behalf of incapacitated individuals in daily personal needs management and decision- making.
  • Conduct intake interview.
  • Assess client needs and wishes
  • Develop action plans that will assist clients in maintaining quality of life and safety within the least restrictive setting.
  • Complete social history
  • Refer to community services
  • Coordinate client services.
  • Monitor client services
  • Reassess client needs and revise plans as needed.
  • Conduct home and facility visits.
  • Advocate with health care facilities and other institutions on behalf of client.
  • Appropriately bill client accounts for services rendered.
  • Maintain and manage all records and reports to ensure accuracy and compliance with policies and procedures.

2.0 Maintains up to date professional knowledge of community services, entitlement benefits, and competencies.

  • Participate in all Lifespan staff meetings and training courses as appropriate.
  • Participate in National Guardianship Association (NGA) sponsored training.

3.0 Other Duties as assigned

QUALIFICATIONS

EDUCATION: Bachelor’s degree in social work, human services or equivalent

EXPERIENCE: Three years in social work or human services. Familiarity with community resources and experience with older

adults preferred.

PHYSICAL AND MENTAL REQUIREMENTS

  • 80% of the work is community based: must be able to meet with individuals in their home environment and in a variety of community settings including hospital and nursing home environments.
  • 20% of the work is performed at a desk or workstation
  • Must be able to work in an open office environment
  • Must demonstrate verbal, written, organizational and leadership skills
  • Must be able to maintain composure under difficult and challenging situations
  • Must be able to work in multi-cultural situations
  • Must have valid NYS driver license and reliable transportation
  • Must demonstrate working knowledge of Excel, word, outlook, internet web-based programs, and web applications
  • Normal daily walking and sitting.
  • Frequent computer usage.
  • Some lifting (files, etc.) <20 lbs.
