Monday–Friday | 40 hours per week

On-Call Rotation (1 Weekend/Month plus coverage as needed)

Position Overview:

The Washington County Commission on Aging, Inc., is seeking a dedicated and compassionate professional for a dual-role position serving as both a Guardianship Case Manager and Dementia Care Specialist.

This full-time role is evenly split:

50% Guardianship Case Management – Providing advocacy, coordination of care, and oversight for individuals under public guardianship.

Key Responsibilities:

  • Make regular in-person visits to clients (at home, hospitals, or care facilities).
  • Monitor health, safety, and overall well-being.
  • Collaborate with healthcare providers, legal representatives, and service agencies.
  • Provide informed consent for medical treatments and living arrangements as needed.
  • Maintain accurate documentation and timely reporting for court and agency records.
  • Attend court hearings and team meetings as required.

· Additionally, this position includes on-call responsibilities one weekend per month and provides coverage for time off for the full-time Guardianship Case Manager.

Key Responsibilities:

50% Dementia Care Specialist – Offering person-centered assistance, resource navigation, and intake services for community members seeking access to dementia, caregiver, aging, and disability support services.

· provide new or existing services for person with symptoms of dementia that interfere with activities of daily living and their caregivers

· Provide information and referrals to long-term care and chronic disease services.

· Participate in outreach events connecting with persons living with dementia and their caregiver’s quarterly.

· Encouraging and providing programs that engage individuals in regular exercise, and social activities

Qualifications:

  • Required: Associate’s Degree in Human Services or related field
  • Preferred: Bachelor’s Degree in Human Services, Social Work, or a related field
  • Experience:
  • Case management experience required
  • Strong documentation and computer skills essential

· Excellent communication and interpersonal abilities

· Demonstrated ability to learn quickly and work independently in a fast-paced environment

· Must possess a valid driver’s license and have reliable transportation with current auto insurance

· Must pass a criminal background check, physical and drug screen

· Vaccines are required for this position

Compensation & Benefits:

  • Salary Range: $38,000
  • Health, dental, and vision insurance (single & family coverage available).
