Job Summary
The Habilitation Manager is a key member of our agency’s internal habilitation team, serving as the central coordinator for member habilitation goals under Arizona DDD programs. Beyond collaboratively developing, monitoring, and refining person-centered habilitation plans, this role oversees operational processes within the HCBS teams, ensuring that workflows, documentation, and quality control align with best practices and DDD standards. The Habilitation Manager ensures smooth, efficient operations while maintaining consistency, quality, and compliance in all aspects of service planning and delivery. This role also drives innovation by designing and implementing creative training methods for field staff, equipping them to deliver habilitation goals effectively and consistently in the community.
Key Responsibilities

Assists in the design and implementation of person-centered habilitation plans in collaboration with support coordinators, habilitation specialists, lifecare supervisors, and families.

Supports internal agency operations related to habilitation services, including workflow coordination, process efficiency, and resource allocation.

Prepare and monitor applicable reports monthly, quarterly and annually as requested by the VP of Operations and the President and CEO.

Facilitate internal team coordination ensuring direct support providers receive clear guidance and goal direction.

Attends regular DDD progress reviews and team meetings; use data insights to optimize both client outcomes and internal processes.[SW1]

Assists with training, support, and guidance to frontline staff on agency protocol, documentation standards, and best practices.

Maintains habilitation-related documentation and ensuring accuracy, timeliness, and regulatory compliance.

Monitoring HCSB’s performance via audits, reporting, and feedback loops to support continuous improvement.

Stay current with DDD policies and HCBS regulations, applying these consistently across agency operations

Proactive community engagement, actively facilitating outings and activities.
Qualifications

High School Diploma or GED (required)

Bachelor’s degree in human services, psychology, art therapy, speech pathology or a related discipline (highly preferred).

Minimum of 2 years of experience in training/coaching, working with individuals with disabilities.

Familiarity with Arizona Habilitation regulations for the Department of Developmental services.

Experience working with or within HCBS/DD systems; ABA knowledge and habilitation service development strongly preferred

Demonstrated proficiency in coordinating habilitation or support services, with exposure to operational or administrative responsibilities.

Strong organizational, interpersonal, and communication skills.

Valid Arizona driver’s license and reliable transportation.

Arizona Level I Fingerprint Clearance Card or willingness to obtain.

CPR and First Aid certifications, or willingness to obtain
