HCS Case Manager

Divine Comfort Homecare Service is a Medicaid Waiver program provider for individuals with intellectual disability. We specialize in Home and Community Services (HCS) and Texas Home Living Medicaid Waiver services. The role of the HCS Case Manager will include but not limited to the following:

Client Intake

Needs Assessment

Quarterly and Annual Service Planning Meetings

Monitoring and Evaluation of Client Progression

Scheduling of Client Appointments

Documentation and Maintenance of Client Records

Pay: $15.00 - $20.00 per hour

Experience:
