Health Care Coordinator (Nurse)

ID/DD Wellness Nurse

Position Summary:

This position is responsible for the provision of Wellness Coordination Services to manage health care needs of Intellectually Disabled / Developmentally Disabled individuals. The position requires attention to detail, accurate, thorough and timely documentation, a commitment to individual services and a team mentality.

Reports To: Executive Director (as to programmatic issues) President (as to nursing)

Experience and Qualifications:

  • At least 25 years of age
  • High School Diploma or GED equivalency
  • CPR certification
  • Free from communicable diseases
  • Valid Indiana drivers license;
  • Owns or has access to a vehicle in good working order;
  • Current automobile insurance (for vehicles registered in the state of Indiana);
  • Clean criminal history and OIG record
  • No findings on any aide registry
  • Successful completion of Training and Orientation program

  • Ability to document in a timely and accurate manner according to principles of documentation
  • Excellent interpersonal and communication skills
  • Ability to accomplish tasks with minimum supervision in a timely manner
  • Strong observation, organization and problem solving skills
  • Knowledge of Medicaid and Medicaid Waiver rules and regulations, particulary those that apply to providers of Waiver Services
  • Ability to interact successfully with a variety of individuals who function on many different levels including individuals, their guardians and family members, Supervisors, other professionals, Direct Support Professionals, Case managers, Behavior consultants, Physicians and State and or Federal personnel.
  • Basic knowledge of processing and Word software

Position Physical Requirements:

Position requires the ability to walk, sit, stand; lift, push and pull up to 50 pounds unassisted and 350 pounds with assistance; squat, bend, reach and stretch.

Supervisory Responsibilities: No.

Key Responsibilities and Duties:

  • Coordinate Wellness Coordination
  • Complete Wellness Assessment and a Wellness Plan for each
  • Measure the following outcomes data points;
    • Height, Weight and BMI
    • Annual flu vaccinations
    • Annual physician/exam
    • Annual dental visits
    • Five (5) percent decrease in the number of medication administration errors in incident reports over a 12 month period
    • Five (5) percent decrease in the number of choking incidents that require intervention over a 12 month period
    • Five (5) percent decrease in the number of falling incidents that require intervention over a 12 month period
  • Monitor Tier I: Health care needs require at least weekly* consultation/review with RN/LPN, including face-to-face visits once a
  • Monitor Tier II: Health care needs require at least weekly* consultation/review with RN/LPN, including face-to-face visits twice a
  • Monitor Tier III: Health care needs require at least weekly* consultation/review with RN/LPN, including face-to-face visits once a week

Assessments and Data Collection

In addition to meeting service definition requirements, nurse must coordinates health care services, completes wellness assessments and a wellness plan for each individual. Information collected through these assessments is important for the development of the individual’s wellness plan, which drives his/her medical and health goals toward positive outcomes. These assessments and the plan are required to ensure the best medical and health coordination for each individual, and hold the provider accountable for the service as a component of its re-approval process.
