Our Operations team works closely with Clinical, Sales, and Finance to ensure patients are admitted timely, documentation is accurate, and processes are followed consistently.

Key Responsibilities

Intake & Care Coordination Support

  • Support daily intake and care coordination workflows
  • Review referrals and patient records for completeness, accuracy, and timeliness
  • Ensure documentation meets internal standards and deadlines
  • Identify missing, incorrect, or delayed items and route for follow-up
  • Coordinate with intake, clinical, billing, and administrative teams

Operational Review & Process Support

  • Monitor assigned workflows to ensure procedures are followed consistently
  • Perform routine quality checks on documentation and operational tasks
  • Identify recurring issues or gaps and escalate appropriately
  • Maintain trackers, reports, and internal documentation
  • Support implementation of updated workflows and SOPs

Compliance & Accreditation Support

  • Ensure records and workflows align with internal compliance standards
  • Assist with internal audits and documentation reviews
  • Support preparation for accreditation and survey activities (e.g., JCAHO, DPH)
  • Maintain organized, audit-ready records

QualificationsRequired

  • Bachelor’s Degree
  • 3 years of experience in healthcare coordination, intake, or operations support
  • Strong attention to detail and ability to manage multiple tasks
  • Process-oriented mindset with ability to follow and validate procedures
  • At least 1 year of experience coordinating with remote teams
  • Strong organizational and problem-solving skills
  • Clear written and verbal communication skills
  • Comfortable navigating and working within EMR systems
  • Demonstrated working knowledge of Microsoft Office (Word, Excel, Outlook)
  • Ability to work onsite with minimal supervision
  • Availability to work one weekend per month, as needed

Preferred

  • Home Health experience
  • Exposure to compliance, audits, or regulated healthcare environments
  • Clinical background (preferred, not required)

Compensation & Benefits

  • Pay Range: $55,000 – $60,000
  • Bonus: Up to $5,000 annually, based on performance and company results
