Health Home Aide in Federalsburg, Baltimore County, Princess Anne County

Job Overview
We are seeking compassionate and dedicated Home Health Aides, CNAs, and GNAs in Prince Georges, Montgomery, Howard Counties to provide exceptional in-home and community-based care to diverse patient populations. This role involves supporting individuals with various health needs, including seniors, those with disabilities, and individuals requiring specialized care such as Alzheimer's or dementia care. The ideal candidates will be committed to promoting independence, dignity, and quality of life for their clients while adhering to all healthcare standards and regulations.

Duties

  • Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
  • Monitor and observe patient conditions, reporting any changes or concerns promptly
  • Administer medications and treatments as prescribed under supervision, ensuring proper documentation
  • Utilize Electronic Health Record (EHR) or EMR systems for accurate medical documentation and record keeping
  • Perform vital signs assessments such as blood pressure, pulse, temperature, and respiration rates
  • Support patients with meal preparation and nutritional needs, ensuring dietary restrictions are followed
  • Assist with catheter care, wound care, and other specialized medical procedures when trained
  • Operate medical equipment such as Hoyer lifts safely and effectively
  • Provide companionship and emotional support to promote mental well-being
  • Maintain a clean and safe environment by assisting with light housekeeping tasks like laundry and cleaning

Requirements

  • Valid certification as a Home Health Aide (HHA), CNA, or GNA; relevant experience preferred but not required
  • Experience working in assisted living facilities, nursing homes, hospice care, or home health settings is highly desirable
  • Knowledge of medical terminology and patient care best practices
  • Ability to work with individuals with developmental disabilities, Alzheimer's disease, dementia, or memory care needs
  • CPR certification required; additional certifications such as medication administration or hospice care are a plus
  • Familiarity with HIPAA regulations to ensure patient confidentiality and privacy
  • Strong communication skills for effective interaction with patients, families, and healthcare teams
  • Ability to work independently in home settings or community environments
  • Physical ability to assist with patient mobility using equipment like Hoyer lifts; capable of lifting up to 50 pounds when necessary
  • Compassionate attitude with a genuine desire to improve the quality of life for others
