Overview:
Preps, scans and/or performs quality review on patient charts entered into the optical record system for permanent retention.
Responsibilities:
- Pick up inpatient and same day charts from all nursing units and maintains accountability for charts from all nursing units within 48 hours of discharge.

Accurately prepares charts for scanning. Responsible for document entry via all scanning equipment and uploading from shared folders via the shared drive.

Performs Indexing on all charts and transfer data to permanent medical record.

Required skills must include a thorough working knowledge of chart content and chart order to enable assembly of a quality product of all patient type records.

Must be able to identify scanner, printer and fax equipment problems and seek immediate resolution.

Flexibility in scheduling may be necessary based on patient volume and patient care needs.

Release patient information to nursing units and other healthcare facilities as appropriate for continuation of care.

Reconciles Scanning/Indexing worklists in Expanse.

Maintain office area to include stocking supplies, cleaning, keeping the area organized, making appropriate person aware of needed supplies and/or services, and working with others to ensure shift flow.

Qualifications:
- High School diploma or equivalent.

Typing and filing skills.
- Additional specialized courses or training in medical records preferred.

Must have strong personal computer skills.
- Exhibits interpersonal skills and abilities to deal effectively with all levels of staff.
- Must be detailed oriented, self-motivated and have the ability to stay focused on tasks for extended periods of time.

Must be flexible and have the ability to adapt to stressful situations and work schedules.

Must be able to read, speak and write English.
