Prestige Development Group is seeking experienced Non-Clinical Health Care Workers (Case Managers) to manage full lifecycle case coordination in a remote environment.This role focuses on intake validation, user outreach, benefits application assistance, submission verification, case tracking, and escalation of high-risk cases. The position supports individuals applying for programs such as Medicaid, SNAP, LIHEAP, housing assistance, and other social services. This is not a clinical position and does not involve medical decision-making. Key Responsibilities

  • Review and validate intake records from referral sources and automated systems
  • Contact users to verify identity and clarify service needs
  • Assist individuals with applications for Medicaid, SNAP, LIHEAP, housing assistance, and other public benefits
  • Ensure applications are complete and submitted accurately
  • Confirm submission and maintain case documentation in the CRM system
  • Track cases against service-level agreement (SLA) timelines
  • Escalate urgent cases involving:
  • Utility shutoff risk
  • Food insecurity
  • Durable medical equipment (DME) needs
  • Vulnerable or high-risk populations
  • Follow established workflows, escalation procedures, and documentation standards

Qualifications Required

  • 3+ years of experience in healthcare coordination, case management, benefits navigation, or social services
  • Strong familiarity with Medicaid and public assistance programs
  • Experience working with high-risk or elderly populations
  • Strong documentation and organizational skills
  • Experience working with CRM systems (Salesforce preferred)
  • Excellent English communication skills (written and verbal)

Preferred

  • Spanish English bilingual
  • Experience with hospital discharge coordination
  • Experience managing durable medical equipment (DME) cases
  • Experience working in SLA-driven environments

  • This is a metrics-driven role. Performance will be evaluated based on:
  • Time to first user contact
  • SLA compliance rate
  • Submission accuracy
  • Documentation completeness
  • Escalation appropriateness
  • Case resolution timelines
  • Accuracy and documentation integrity are prioritized over speed.

Compensation

  • Contract Role Hourly Rate (Candidate Proposed)

  • Language proficiency
  • Workflow discipline
