The Healthy Start Care Coordinator supports the health and well-being of pregnant and parenting women, fathers/partners, infants, and children (up to 18 months) through the Healthy Start program. This role primarily provides one-on-one care coordination using a family-centered, strength-based, and trauma-informed approach.
- Engages in community outreach efforts, including canvassing, events, partnerships, targeted outreach, and other activities identified in the Healthy Start outreach plan to recruit and retain program participants.
- Provides individualized care coordination, including education, system navigation, referrals to community resources, and follow-up to ensure successful access to services.
- Conducts visits in participant homes, community settings, agency offices, or through virtual/phone formats.
- Conducts screenings and develops, implements, and updates individualized care plans; monitors progress and adjusts strategies accordingly. Supports participants in navigating healthcare and social service systems, including scheduling appointments and removing potential barriers.
- Offers education using evidence-based health and parenting curricula, as identified by the program.
- Uses interpretation or translation services as needed to ensure accessible care.
- Provides support to Healthy Start group-based health and parenting education sessions as needed, including creating a safe, welcoming environment that promotes learning and peer connection.
- Coordinates and/or provides group logistics, including enrollment, transportation (using an agency vehicle), childcare, and interpretation.
- Conducts individualized follow-up support as needed for group participants.
- Represents the program in the Healthy Start Consortia and community collaborations as assigned.
- Maintains timely, accurate, and confidential documentation in compliance with program standards. Participates in team meetings, supervisions, trainings, and agency meetings as assigned.
- Manages time, schedule, and workload responsibilities effectively.
- Participates in quality improvement projects.
- Engages in ongoing professional development.
- High School Diploma/GED with 2 years of experience with young children, parents and/or families or an associate degree is required; Bachelor’s degree is preferred
- Ability to work respectfully with all populations
- Ability to effectively engage with clients and community partners about program services
- Good working knowledge of community resources and accessing these resources preferred
- Complies with accepted professional standards and practice
- Ability to work effectively with a multi-disciplinary team
- Ability to work with the management information system and complete data entry
- Mandatory Reporter of Child and Dependent Adult abuse training certification (within 30 days of hire)
- Must have valid Iowa driver's license, auto liability insurance and reliable transportation if driving is required
- Successful completion of pre-employment physical, drug screen, 2-step TB test and background check
- Physical requirements: Light work, sitting most of the time, exerting up to 20 pounds of force frequently, and/or a small amount of force constantly to move objects.

- Time: Ability to be flexible with a varying schedule and programmatic requirements
- Travel: Required between locations
