Join our compassionate team as a Home Health Aide (HHA) and Certified Nursing Assistant (CNA), where your dedication will make a meaningful difference in the lives of individuals requiring personalized care. In this energetic and rewarding role, you will provide essential support to patients in various settings, including assisted living, nursing homes, and home care environments. Your expertise in patient observation, medication administration, and assisting with Activities of Daily Living (ADLs) will ensure our clients receive the highest quality of care. This position offers a dynamic work environment where your caregiving skills help promote independence, comfort, and dignity for those you serve.

Responsibilities

  • Deliver attentive patient observation to monitor health status and report changes promptly.
  • Assist with ADLs such as bathing, dressing, grooming, toileting, and mobility support using tools like Hoyer lifts when necessary.
  • Manage medical documentation accurately within Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to ensure compliance with HIPAA regulations.
  • Support medication administration following proper protocols and medical terminology understanding.
  • Perform vital signs assessments including blood pressure, pulse, temperature, and respiration rates to track patient health trends.
  • Provide specialized dementia and Alzheimer’s care, including memory care techniques to enhance patient comfort and safety.
  • Assist with catheter care, hospice support, and other complex medical needs as trained.
  • Prepare nutritious meals and assist with feeding while adhering to dietary restrictions.
  • Maintain a clean environment by assisting with laundry, cleaning duties, and ensuring infection control standards are met.
  • Use medical equipment such as Hoyer lifts safely and effectively to prevent injury.
  • Document all patient interactions meticulously in medical records for continuity of care.
  • Support home & community care initiatives by helping clients maintain independence in their daily routines.

Qualifications

  • Valid certification as a Home Health Aide (HHA) or Certified Nursing Assistant (CNA).
  • Proven experience in long-term care settings such as nursing homes or assisted living facilities.
  • Familiarity with working with individuals with developmental disabilities and dementia-related conditions like Alzheimer’s disease.
  • Knowledge of medical terminology, medication administration procedures, and vital signs monitoring.
  • Experience using EMR/EHR systems for accurate documentation of patient records.
  • CPR certification required; additional certifications such as hospice care or catheter care are a plus.
  • Ability to work independently while following detailed care plans; excellent communication skills are essential.
