Qualifications:
- Graduate of a State of Connecticut accredited program for licensed practical nurses.
- A current Connecticut LPN license and CPR certification.
- Demonstrate relevant acute care or sub-acute care clinical experience.
- Effective communication skills.
- Computer competency.
- Strong organizational and time management skills.
- Experience with end-of-life care for hospice patients
Responsibilities:
Patient Care, following the plan of care and supervision of the primary care nurse (PCN)
- Delivers skilled nursing care and treatments in a compassionate, patient-centered manner according to physician orders, and within scope of practice, as directed by PCN and/or IDG for hospice patients
- Assists the RN in performing specialized procedures and duties which include preparing equipment and materials for treatment, observing appropriate infection control practices, and assisting the patient in learning appropriate self-care techniques
- Coordinates with PCN to prioritize the needs of the patients for the day.
- Assists the PCN and/or IDG for hospice patients in carrying out the plan of care.
- Notifies the physician, PCN and the supervisor timely regarding significant changes in the patient’s status.
- Completes documentation of visit notes, case communications and orders on a timely basis
- Assists the patient in learning appropriate self-care techniques.
- Reports to PCN at the end of each workday/shift regarding the status of each patient visited including plan for next visit and updates to visit schedule to meet patient needs..
- Participates in health promotional activities such as blood pressure and influenza clinics and the delivery of telehealth services or other agency services within scope of practice and as assigned.
Educational
- Attends yearly OSHA in-service.
- Identifies knowledge gaps and remedies this independently.
- Attends minimum 12 hours of in-services per year, including end-of-life care topics..
- Shares information with other Agency personnel.
