High Needs Case Manager

Trinity Integrated Care is seeking a High Needs Case Manager to work between various locations in Maricopa County, including various shelters. The High Needs Case Manager will be responsible for providing case management services to members seeking services and housing opportunities. The Case Manager will work closely with members to identify their needs, connect them to appropriate services, and support them in achieving their objectives.

Responsibilities:

• Provide case management services to members seeking services and housing opportunities.
• Collaborate with members to assess their individual housing needs and develop a personalized plan to help them obtain and maintain suitable housing.
• Identify appropriate resources and services to support members' housing needs, and facilitate access to available housing opportunities.
• Provide ongoing support to members as they work towards achieving their objectives, and help them overcome any barriers that may arise along the way.
• Conduct assessments to determine members' needs, strengths, and objectives.
• Develop and implement individualized care plans with members to achieve their objectives.
• Provide referrals and linkages to appropriate community resources and services.
• Monitor and track members' progress towards achieving their objectives.
• Collaborate with other service providers to coordinate care and services.

Qualifications:

• Bachelor's degree is preferred
• Minimum of 2 years of experience in case management, preferably in a homeless or housing-related setting.
• Experience working with individuals who have high needs and complex issues, such as mental health, substance use, and chronic medical conditions.
• Strong knowledge of community resources and services available to individuals experiencing homelessness.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Bilingual in Spanish and English is preferred.
• Must have experience identifying housing opportunities.
