Hiring an Advanced Practice Provider for Out-Patient Primary Care

Job Overview
We are seeking a highly skilled and dedicated Physician Assistant or Family Nurse Practitioner to join our dynamic healthcare team. The ideal candidate will possess comprehensive clinical expertise, a passion for patient-centered care, and have experience in primary care.

Key Responsibilities

  • Provide comprehensive primary care services including preventive care, acute visits, and chronic disease management
  • Strong knowledge of medical terminology, anatomy, physiology, and pharmacology necessary for effective patient assessment and treatment planning.
  • Diagnose and manage common primary care conditions such as hypertension, diabetes, hyperlipidemia, COPD/asthma, obesity, chronic pain, sleep disorders, thyroid disorders, and mental health conditions, among others
  • Perform and interpret routine diagnostic tests (labs, EKGs, ABIs)
  • Prescribe medications and develop evidence-based treatment plans
  • Document encounters accurately and efficiently within the electronic medical record
  • Educate patients on disease prevention, lifestyle modification, and treatment adherence
  • Maintain continuity of care through longitudinal patient relationships, including appropriate follow-ups and monitoring of treatment response
  • Conduct annual wellness visits, health risk assessments, and age-appropriate preventative screenings
  • Manage transitional care following hospitalizations or emergency department visits
  • Review, interpret, and act on laboratory results, imaging studies, and diagnostic reports in a timely manner

Schedule & Practice Environment

  • Full-time outpatient position
  • No inpatient responsibilities
  • No call requirements
  • Supportive physician supervision and collaborative team structure
  • Dedicated clinical and administrative support staff
  • Monday-Friday, 8:00 a.m. - 4:00p.m.
  • Focus on quality, continuity, and patient-centered care—not volume-driven medicine

Compensation & Benefits

Required Qualifications:

  • Minimum 2 years of primary care or internal medicine experience
  • Current, unrestricted state license
  • National certification (PA-C, FNP, or AGPCNP)

Preferred Qualifications

  • Experience with Practice Fusion and/or Advanced MD
  • Dermatology experience
  • Excellent communication, organization, and time-management skills

Job Type: Full-time

Pay: $120,000.00 - $135,000.00 per year
