Job Overview
We are seeking a highly skilled and dedicated Physician Assistant or Family Nurse Practitioner to join our dynamic healthcare team. The ideal candidate will possess comprehensive clinical expertise, a passion for patient-centered care, and have experience in primary care.
Key Responsibilities
- Provide comprehensive primary care services including preventive care, acute visits, and chronic disease management
- Strong knowledge of medical terminology, anatomy, physiology, and pharmacology necessary for effective patient assessment and treatment planning.
- Diagnose and manage common primary care conditions such as hypertension, diabetes, hyperlipidemia, COPD/asthma, obesity, chronic pain, sleep disorders, thyroid disorders, and mental health conditions, among others
- Perform and interpret routine diagnostic tests (labs, EKGs, ABIs)
- Prescribe medications and develop evidence-based treatment plans
- Document encounters accurately and efficiently within the electronic medical record
- Educate patients on disease prevention, lifestyle modification, and treatment adherence
- Maintain continuity of care through longitudinal patient relationships, including appropriate follow-ups and monitoring of treatment response
- Conduct annual wellness visits, health risk assessments, and age-appropriate preventative screenings
- Manage transitional care following hospitalizations or emergency department visits
- Review, interpret, and act on laboratory results, imaging studies, and diagnostic reports in a timely manner
Schedule & Practice Environment
- Full-time outpatient position
- No inpatient responsibilities
- No call requirements
- Supportive physician supervision and collaborative team structure
- Dedicated clinical and administrative support staff
- Monday-Friday, 8:00 a.m. - 4:00p.m.
- Focus on quality, continuity, and patient-centered care—not volume-driven medicine

Required Qualifications:
- Minimum 2 years of primary care or internal medicine experience
- Current, unrestricted state license
- National certification (PA-C, FNP, or AGPCNP)
Preferred Qualifications
- Experience with Practice Fusion and/or Advanced MD
- Dermatology experience
- Excellent communication, organization, and time-management skills
Job Type: Full-time
Pay: $120,000.00 - $135,000.00 per year
