Job Overview
We are seeking a dedicated and compassionate Licensed Vocational Nurse (LVN) to join our Blue Sky Home Health healthcare team. The ideal candidate will provide quality care to patients, ensuring their comfort and well-being. This role involves working closely with RNs, the DPCS (Director of Patient care), the case managers, the office manager, the Administrator, and the office staff.environment.

Location / Coverage area:

Antelope Valley and surrounding areas. For example: Santa Clarita, Palmdale, Lancaster, Quartz Hill, Rosamond, Littlerock, Pearlblossom, Lake Los Angeles, Mojave, and California city.

Role Description

This is a per diem remote role for a Licensed Vocational Nurse (LVN) with previous home health experience. The LVN will be responsible for providing skilled nursing care to patients in their homes. Primary responsibilities include assessing the patient's health status, administering medications, managing pain, and educating patients and their families about healthcare needs and treatments.

Compensation:

Per-diem compensation based on home health experience:

Range: $45 - $55 per visit, based on nursing skill set, and previous home health experience.

Please note that all new hires are required to successfully complete a 90-day probationary period!

Duties

• Administer medications and treatments as prescribed by physicians.
• Monitor patients’ vital signs, such as blood pressure, heart rate, and temperature.
• Assist in patient care planning and follow up on patient progress.
• Perform wound care, IV administration, and other necessary procedures.
• Educate patients and families on care plans and post-treatment needs.
• Maintain accurate patient records and documentation.
• Ensure the cleanliness and safety of the clinical environment.
• Support physicians and other medical staff during examinations and procedures.

Qualifications

• Valid LVN license in the state of California.
• Current CPR/BLS certification.
• Minimum 1 year of previous Home Health experience
• Experience in a clinical or long-term care setting preferred.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Excellent organizational skills and attention to detail.
• Ability to handle multiple tasks and prioritize effectively.
• Spanish language skill is a plus!
