Job Overview:

We are seeking a dedicated, compassionate, and bilingual Registered Nurse (RN) to join our healthcare team. The ideal candidate will provide quality care to patients, ensuring their comfort and well-being. This role involves working closely with LVNs, the DPCS (Director of Patient care), the case managers, the office manager, the Administrator, and the office staff.environment.

We are looking for RNs who are able to cover the following areas: Hemet, Riverside, Temecula, Murrieta, San Bernardino, Corona, Uplands, Rancho Cucamonga, Perris, Winchester, Menifee, Lakeview, Redlands, Fontana, Colton.

Role Description

This is a per-diem remote role for a Registered Nurse. The Registered Nurse will be responsible for providing skilled nursing care to patients in their homes. Primary responsibilities include assessing the patient's health status, administering medications, managing pain, and educating patients and their families about healthcare needs and treatments.

Compensation:

• Per-diem compensation! Compensation is based on home health experience, skill set, type of nursing service provided, and patient location.
• Rate range: $100 to $150 per visit.
• PLEASE note that all new hires are required to successfully complete a 90-day probationary period.

Responsibilities

- Assess and monitor patient health status, including vital signs and medical history.
- Administer medications and treatments as prescribed by physicians.
- Provide care for patients requiring dialysis and assist in ventilating patients when necessary.
- Conduct comprehensive assessments using the Minimum Data Set (MDS) for appropriate care planning.
- Implement infection control protocols to ensure patient safety.
- Collaborate with interdisciplinary teams to develop and execute individualized care plans.
- Educate patients and families on health management, including family planning and ophthalmology concerns.
- Participate in critical care transport as needed, ensuring safe transfer of patients.
- Maintain accurate documentation of patient care activities, coding, and treatment outcomes.

Qualifications

a) Home Health Experience is a MUST

⁠ ⁠b) Current and unrestricted RN license in the state of practice

⁠ ⁠c) Minimum of one-year nursing experience in an acute care setting

⁠ ⁠d) Excellent clinical knowledge and critical thinking skills

⁠ e) ⁠Wound Care skills and IV therapy care skills are a plus

⁠ ⁠f) Knowledge of PICC line treatment is a plus

⁠ ⁠g) Demonstrated ability to manage multiple priorities and complex patient care

⁠ ⁠h) Strong communication and interpersonal skills

⁠ ⁠i) Ability to work independently and remotely

⁠ ⁠j) Experience in home health care is preferred

⁠ ⁠k) Bachelor's degree in Nursing or related field is preferred

l) Bilingual language skills
