Job Description

AccordCare is a licensed agency seeking experienced High-Tech Per Diem Field RNs and LPNs to provide skilled care throughout Broward County.

We are hiring RNs and LPNs for high-tech care visits, including patients requiring:

• Ventilator/Trach care

• PEG tube management

• Wound care

• IV therapy

Experience with Paraplegic, Quadriplegic, and TBI care is a plus.

Position Summary

The RN or LPN provides the highest quality of care to patients in accordance with their Plan of Care, ensuring patient safety, well-being, and satisfaction.

Key Responsibilities

• Bilingual Spanish/English preferred

• Travel throughout Broward County, outside of Coral Springs as needed

• Provide safe and effective personal care based on the individualized Plan of Care

• Transport clients to and from appointments/activities safely as requested

• Communicate promptly with Nurse Supervisor regarding changes in patient condition or emergencies

• Deliver skilled nursing and personal care within scope of practice, including:

  • Medication administration

  • Transferring and positioning

  • Catheter care

  • Communication with client, family, and Nursing Supervisor

• Occasionally perform client assessments and supervisory visits

• Demonstrate empathy, professionalism, attention to detail, and strong follow-through

• Anticipate potential issues and appropriately escalate or resolve concerns

• Prioritize tasks and respond with a sense of urgency

Required Experience & Education

• Active Florida RN or LPN license

• CPR Certification

• Current Level 2 Background Screening in the AHCA Clearinghouse

• TB test within the last 12 months or Chest X-ray within the last 5 years

• HIV and DOEA Alzheimer’s training certificates (CEUs)

• Prior home care experience

• Excellent communication, teamwork, collaboration, and customer service skills

• Proficiency with electronic health records (EHR) for documentation and care coordination
