Home Based Caseworker

Overview
We are seeking a dedicated and compassionate Case Worker to join our team. The ideal candidate will play a crucial role in supporting individuals and families in navigating complex social systems. This position requires a strong commitment to advocacy, crisis management, and the promotion of well-being for clients, particularly those with disabilities or in vulnerable situations.

Duties

• Conduct assessments to determine client needs and develop personalized service plans.
• Utilize motivational interviewing techniques to empower clients in setting and achieving their goals.
• Provide crisis intervention services to individuals and families facing immediate challenges.
• Collaborate with multidisciplinary teams to coordinate services for clients, including healthcare, education, and social services.
• Manage conflicts effectively while maintaining a supportive environment for clients.
• Work closely with children and families involved in child protective services, ensuring their safety and well-being.
• Advocate for clients within child welfare systems, providing guidance on family law matters as needed.
• Maintain accurate documentation of client interactions, progress notes, and case files in compliance with regulatory standards.
• Stay informed about community resources and support networks available to clients.

Requirements

• Bachelor’s degree in Social Work or a related field; relevant experience may be considered in lieu of degree.
• Proven experience working with children and families, particularly within child welfare or childcare settings.
• Strong understanding of behavioral health principles and crisis management strategies.
• Familiarity with conflict management techniques and the ability to navigate sensitive situations with empathy.
• Experience working with individuals with disabilities is highly desirable.
• Excellent communication skills, both verbal and written, with the ability to build rapport with diverse populations.
• Ability to work independently as well as collaboratively within a team environment.
