Job Summary
We are seeking a dynamic and compassionate Community Liaison Assistant to serve as a vital bridge between community members and our organization. In this role, you will foster strong relationships, facilitate effective communication, and support community engagement initiatives. Responsibilities
- Build and maintain positive relationships with the County Offices of Assistance.
- Coordinate with consumers who are applying for Medicaid. Collect paperwork and prepare applications.
- Schedule assessments for caseworkers with the Philadelphia Corporation for Aging.
- Schedule assessments for the Managed Care Organizations(MCOs).
- Home visits to pick up paperwork like bank statements, identification cards, etc.
Skills
- Proven experience coordinating with the CAOs, PCA and MCOs.
- Strong organization skills.
- Demonstrated ability to work with others.
- Competence in data collection and follow up for program evaluation and reporting purposes
- .Excellent communication skills.
- Background in public health, social work, home care, case management or related fields is highly desirable.
- Leadership qualities with the ability to motivate others and foster teamwork.
- Excellent administrative skills with attention to detail in record keeping and scheduling.
Work Location: In person
Pay: $17.00 - $20.00 per hour
