Home Care Female Registered Nurse (DOH Licensed)

Key Responsibilities:

• Conduct comprehensive assessments of patients in their homes, including physical, emotional, and social evaluations.
• Develop, implement, and regularly update individualized care plans based on physician orders and patient needs.
• Administer medications, injections, and treatments as prescribed.
• Monitor vital signs, symptoms, and changes in condition, and report any significant findings to the physician and care team.
• Coordinate with physicians, therapists, and other healthcare professionals to ensure effective interdisciplinary care.
• Document all nursing visits, interventions, and patient responses accurately and promptly in accordance with agency and DOH (or MOH) standards.
