Home Care Home Health Aide - PRN

Purpose:

Performs personal care in the client residence according to the care plan. The individual is responsible for observing clients, reporting the observations and documenting the services provided.


Responsibilities:

  • Performs activities of daily living (ADL) to include personal care and bath to the client per care plan.
  • Takes and records vitals when ordered. Assists in ambulation and exercise according to the care plan. Performs range of motion and other simple procedures as ordered.
  • Completes appropriate visit records in a timely manner as per EveryStep policy.
  • Meets the safety needs of clients and uses equipment safely and properly.
  • Reports the client’s condition and significant changes to the Home Health RN.
  • Understands and adheres to established EveryStep policies and procedures.
  • Performs household services essential to health care in the home as assigned.
  • Assists with medications that are ordinarily self-administered as assigned.
  • Attends in-service and continuing education programs as scheduled and necessary.
  • Attends client care conferences as scheduled
  • Supports and promotes a culture of compliance and ethical business conduct. Complies with and cooperates fully in all components of the organization's corporate compliance program. Acts in accordance with and adheres to all applicable regulatory and legal guidelines.

Qualifications:

  • Successfully complete a state established or other training program that meets the Medicaid Conditions of Participation and a competency evaluation
  • Must complete/pass competency skills testing
  • Current Mandatory reporter or within 6 months of employment
  • Valid driver’s license and auto insurance.
  • At least 18 years of age.
  • Current Healthcare Provider CPR certification required.
  • Mature, responsible behavior that includes a thorough understanding of HIPAA regulations.
  • Knowledge of the characteristics of elderly, handicapped, or convalescing individual. Demonstrates empathy for the needs of the ill, injured, frail and impaired.
  • Effective written and oral communication skills; Ability to follow and accurately report verbal and written instructions.
  • Ability to work with little direct supervision. Flexibility and cooperation in fulfilling role obligation.
  • Must have valid driver's license, auto liability insurance and reliable transportation if driving is required
  • Successful completion of pre-employment physical, drug screen, 2-step TB test and background check

Working Conditions:

  • Physical requirements: Medium work exerting up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects.
  • Physical activities: Kneeling, Reaching, Talking, Repetitive motions, Standing, Pulling, Walking, Lifting, Fingering (picking, pinching, typing, etc.), Grasping (applying pressure with fingers and palm), Feeling (perceiving size, shape, texture, temperature by touching), Visual acuity (field of vision for analyzing data, viewing computer, using measurement devices, operating machines, determining accuracy or neatness, and/or making general observations)
  • Environment: Hazards (chemicals, infectious diseases), Atmospheric conditions (fumes, odors, dusts, mists, gases)
  • Time: Ability to be flexible with a varying schedule and programmatic requirements
  • Travel: Required between locations
