Home Care Logistics and Scheduling Coordinator

Salary: $24-$26 per hour

Full-time: Monday through Friday, occasional weekend backup coverage

As our Care Logistics Coordinator, you are the "Air Traffic Controller" of our agency. We provide over 800 hour of care per week to Veterans, Private Payers, and Medicaid recipients across RI and Southern MA. Your job is to ensure every client has the right caregiver at the right time will maintaining the highest standards of regulatory compliance.

Key Responsibilities:

• Own the Weekly schedule: You aren't just filling holes you are optimizing routes and "clustering" caregivers to reduce travel time and overhead.

• Clinical Triage: Act as the point of contact for caregivers in the field. Use your clinical knowledge to escalate issues to the Nurse and Care Coordinator or dispatch field support.

• Build Relationships: You understand the needs of clients and caregivers, your experience will help our agency retain our top caregivers and maintain client satisfaction.

• Problem Solving: When a shift is missed you don't just report it-you fix it. You coordinate with administrative team members to ensure the family is updated and happy.

• Data Oversight: Monitor daily operations to ensure our billing and payroll are airtight for VA and Medicaid audits

Requirements:

• Clinical Foundation: Active RI CNA license or LPN preferred. You must understand ADLs, transfers, and basic care plans.

• The "Puzzle" Mindset: You enjoy the challenge of a complex schedule and can think 3 steps ahead.

• Tech Savvy : Proficient in Google Workspace and Homecare software.

• Availability: This is a Monday through Friday position 7AM to 4PM.

• Attention to Detail: Track and monitor caregiver progress and schedule updates in real-time.

• Communication: Must be a clear and effective communicator, bi-lingual Spanish speaker preferred.
