Home Care LPN (Office) $80,000.00, Transition to Hybrid! + 37 PTO Days + Benefits + 401K

Caring Angels Home Care is looking for a Licensed Practical Nurse to conduct Telehealth Visits from 2:00pm to 9:00pm for our Brooklyn office. This position will transition to hybrid after quality performance during training in office. As a Licensed Practical Nurse you will play an integral role in the day to day activities of our center. Job description:

• The LPN responsibilities include conducting 2:00pm to 9:00pm Telehealth patient visits and monitoring their conditions, conducting assesments, etc.

• You should be able to address all the caregivers’ concerns and make appropriate adjustments to the patients’ in-home management plan.

• Monitor medication and insulin, and completing blood pressure, glucose, urine, etc.

• Listening to the concerns of family members and answering their questions.

• Educating caregivers and family on the aftercare or ongoing care of the patient.

• Providing suggestions for improved healthcare to physicians and family members of the patient.

• Monitoring patient recovery and compiling reports.
