Home Care Nurse

Roles and Responsibilities:

• Patient Care
  • Provide direct nursing care to patients at their homes as per the prescribed care plan.
  • Administer medications, injections, and IV fluids as advised by the physician.
  • Monitor vital signs (BP, pulse, temperature, oxygen saturation, etc.) and record findings accurately.
  • Assist patients with personal hygiene, mobility, feeding, and daily living activities.
  • Manage wound care, catheterization, tracheostomy, or other nursing procedures as required.
• Medical Coordination
  • Communicate regularly with doctors, physiotherapists, and family members regarding the patients condition.
  • Report any significant changes in the patient s health status promptly.
  • Follow physician instructions and ensure proper documentation of treatment provided.
• Documentation and Reporting
  • Maintain accurate and up-to-date patient records, nursing notes, and medication charts.
  • Submit daily reports and updates to the home care supervisor or nursing coordinator.
• Health Education and Support
  • Educate patients and caregivers on medication management, diet, hygiene, and rehabilitation exercises.
  • Provide emotional support and reassurance to patients and their families.
• Compliance and Safety
• Adhere to infection control and safety protocols during home visits.
• Maintain confidentiality and professionalism at all times.
• Ensure the safe handling and disposal of medical waste.
