Roles and Responsibilities:
- Patient Care
- Provide direct nursing care to patients at their homes as per the prescribed care plan.
- Administer medications, injections, and IV fluids as advised by the physician.
- Monitor vital signs (BP, pulse, temperature, oxygen saturation, etc.) and record findings accurately.
- Assist patients with personal hygiene, mobility, feeding, and daily living activities.
- Manage wound care, catheterization, tracheostomy, or other nursing procedures as required.
- Medical Coordination
- Communicate regularly with doctors, physiotherapists, and family members regarding the patients condition.
- Report any significant changes in the patient s health status promptly.
- Follow physician instructions and ensure proper documentation of treatment provided.
- Documentation and Reporting
- Maintain accurate and up-to-date patient records, nursing notes, and medication charts.
- Submit daily reports and updates to the home care supervisor or nursing coordinator.
- Health Education and Support
- Educate patients and caregivers on medication management, diet, hygiene, and rehabilitation exercises.
- Provide emotional support and reassurance to patients and their families.
- Compliance and Safety
- Adhere to infection control and safety protocols during home visits.
- Maintain confidentiality and professionalism at all times.
- Ensure the safe handling and disposal of medical waste.
