Job Responsibilities -
1. Patient Assessment and Care Planning
- Initial assessment: Evaluate the patient's health condition by reviewing medical records, conducting physical exams, and speaking with family members.
- Care plan development: Create personalized care plans based on the patient's specific health needs, goals, and preferences. Collaborate with doctors and other healthcare professionals to ensure comprehensive care.
2. Administering Medications
- Medication management: Administer prescribed medications, including injections, oral medications, and IVs, while monitoring for side effects.
- Patient education: Teach patients and family members how to properly administer medications and recognize side effects or complications.
3. Monitoring Vital Signs and Health Status
- Vital sign checks: Regularly monitor and record vital signs (blood pressure, heart rate, temperature, respiratory rate).
- Health monitoring: Track the progress of chronic conditions such as diabetes, heart disease, or respiratory illnesses. Report any changes in the patient’s condition to the healthcare team.
4. Wound Care and Rehabilitation
- Wound dressing and care: Treat and dress wounds, ulcers, or surgical sites, ensuring proper healing and preventing infection.
- Physical rehabilitation support: Assist with mobility, physical therapy exercises, and activities of daily living (ADLs) like bathing, dressing, and eating.
5. Patient Education and Emotional Support
- Health education: Educate patients and their families on disease management, preventive care, lifestyle modifications, and self-care techniques.
- Emotional support: Provide emotional support to patients and their families, helping them cope with the stress of illness or recovery.
6. Documentation and Reporting
- Accurate record-keeping: Maintain detailed records of patient care, treatments, medications, and any observations or changes in health status.
- Communication: Report any significant changes in patient health to the supervising physician or case manager.
7. Collaborating with the Healthcare Team
- Teamwork: Work closely with physicians, physical therapists, occupational therapists, social workers, and other healthcare professionals to ensure the patient’s needs are met.
- Care coordination: Coordinate services and schedule appointments for additional care or specialists as needed.
8. Promoting Safety and Comfort
- Home safety assessments: Identify any safety hazards in the home and recommend changes to reduce fall risks or improve accessibility.
- Comfort and well-being: Ensure that the patient is comfortable, offering pain management, positioning, and helping with mobility as needed.
9. End-of-Life Care (if applicable)
- Hospice care: For patients receiving end-of-life care, the nurse may provide palliative care to manage symptoms, reduce pain, and offer comfort to both the patient and their family.
10. Emergency Response
- Crisis management: Be prepared to handle medical emergencies (e.g., sudden deterioration of health, falls, or acute episodes) and know when to seek immediate medical help.
