Job Description:

Join our compassionate team as a Home Care Therapist, dedicated to making a positive impact on the lives of individuals in the comfort of their homes. As a Home Care Therapist, you will focus on enhancing clients' independence and quality of life through personalized therapy plans.

Key Responsibilities:

1. Conduct in-home assessments to evaluate clients' daily living needs and challenges.

2. Develop and implement customized therapy plans to address clients' specific goals.

3. Provide therapeutic interventions to improve clients' ability to perform daily activities.

4. Collaborate with clients, families, and healthcare professionals to ensure a holistic approach to care.

5. Educate clients and their families on adaptive techniques and equipment for increased independence.

6. Maintain accurate and detailed documentation of assessments, interventions, and progress.

Qualifications:

1. NY State licensure as an Physical Therapist.
2. Strong clinical and communication skills.

3. Ability to adapt therapy plans to diverse client needs.

4. Valid Driver’s License and/or reliable transportation.

5. Valid Liability Insurance

Experience:

Previous experience in home care or a related field is preferred. New graduates with a passion for home care are welcome to apply.
