As a Physical Therapist, you’ll be on an interdisciplinary team that delivers life-changing care to patients across Suffolk County.

Your Impact:

As a Physical Therapist, you’ll deliver skilled, patient-centered therapy in the home or hospice setting, using evidence-based strategies and a compassionate approach.

You will:

Perform comprehensive evaluations and create individualized care plans

Apply chronic disease management and health promotion strategies

Use motivational interviewing and telehealth tools as needed

Collaborate with interdisciplinary teams and update physician-directed care

Maintain accurate and timely documentation, including OASIS

Coordinate visits and contribute to care planning that prevents re-hospitalization

Educate patients and families to support optimal independence and quality of life

What You Bring:

Licensed and registered as a Physical Therapist in New York State

Ability to meet all NYS and VNSHS compliance requirements

Prior home care experience is a plus — but not required
