Home Health Aide

DUTIES

Comply with applicable laws and regulations.

Provide home care services in the patient's residence, as allowed in Pennsylvania Title 28 Chapter 611, and directed per the care/service plan.

Per the care/service plan, perform personal care, companion services, respite care, specialized care, and/or assistance with instrumental activities of daily living, as trained.

Observe the patient's health condition and promptly notify/report to the supervisor any changes in condition or respond immediately to an emergency, if appropriate, per the care/service plan.

Reinforce patient and family education regarding activities of daily living, care or treatment, diet, activity, disease management, and safety as directed by professional staff.

Maintain patient care records and accurately document provided services in a timely and complete manner within the specified timeframe defined by the Agency.

Provide companionship and basic emotional or psychological support.

Respect and protect the patient’s rights, including refusal of services, confidentiality, and privacy.

Keep Administration, Management, and staff informed of current organizational, community, and industry trends.

Seek and encourage input from other administrative and service personnel.

Represent the organization to other groups, agencies, and the general public as requested.

RESPONSIBILITIES

Participate in the development of the patient care/service plan.

Assist with facilitating appropriate and adequate staffing.

Participate in staff development, including orientation, in-service education, and continuing education as required.

Inform supervisor of any concerning visit observations and any status changes in the patient.

Attend and be punctual for patient visits/services and Agency meetings, education, training, or patient conferences.

Assist and provide input to nursing, the Supervising Physician or Registered Nurse, and other staff, management, and program areas for appropriate services operations and administration.

Assist with submission and completion of documentation of services in accordance with laws and regulations.

Keep the Administrator and other management staff informed of Agency and community developments, needs, etc. Encourage their participation in problem solving at the organization and community levels.

Assist in the evaluation of community needs and plan appropriate programs/services.

Act as the Agency representative in providing services according to the Mission, Vision, and Philosophy.

Participate in selective conferences and workshops to promote knowledge in community health trends, job performance efficiency, and overall professional growth.

Establish personal goals and objectives for advancement.

Participate with other health, civic, educational, and professional groups as directed.

Represent the Agency in community and civic organizations on occasion.

Assist and participate in organizational improvement activities as requested.

JOB CONDITIONS

The position requires meeting deadlines and requires patient care visits in their homes.

The position requires lifting patients and moving home furniture and equipment.

Home visits will involve sitting, standing, bending or stooping, and walking, as well as telephone communication.

Travel is required by car to patients’ homes.

This position requires the ability to communicate in English well, both verbally and in writing.

EQUIPMENT OPERATION

The position requires the ability to use a PC or tablet, calculator, cell phone, multi-line telephone, and other related office equipment.

COMPANY INFORMATION

The Direct Care Worker has access to all patient medical records as needed to provide home care services.

QUALIFICATIONS

  • Education with a minimum of a high school diploma or GED.
  • Ability to demonstrate self-confidence and positive attitude toward self and others and to maintain commitment and enthusiasm to goal achievement.
  • Pass an approved competency exam or certification based on a training program meeting Pennsylvania Title 28 §611.55.
  • Ability to develop and implement job and/or professional improvements.
  • Ability to identify and evaluate personal strengths and weaknesses of self and others.
  • Ability to objectively direct or supervise individuals without personal bias.
  • Must have a current First Aid certification or Basic Life Support (BLS) certification if required by the State.
  • Pass Pennsylvania State Police and Federal criminal history record checks.
  • Pass Child Abuse Clearances according to Acts 33 and 34/ChildLine

Pay: $12.00 - $15.00 per hour
